Additional Required Forms All deaf or hard of hearing applicants must submit the Application for Undergraduate Admission, one reference and an audiogram, regardless of whether you are applying to a NTID program of study or to a program in one of the other colleges of RIT. One Recommendation: If your school counselor completes the recommendation section on the secondary school transcript request form, then that counts as the one letter. Or, if you prefer, a letter can be mailed separately. Audiogram: You can use either the audiological record form (to the right in this chart) or simply send a recent audiogram without the form. | Print Recommendation (PDF) Print Audiogram (PDF) |
